Most people don't plan months ahead to have their wisdom teeth out. A jaw aches on a Tuesday, an X-ray at a regular dental cleaning catches an impacted tooth, or a college student comes home for break with a deadline before the next semester. Who needs wisdom teeth removed?
Not everyone. About a third of adults never need wisdom teeth removal — their teeth come in straight, leave room, and stay clean. The other two thirds run into one or more of the following:
- Impaction — the tooth can't fully emerge because it's blocked by bone or another tooth
- Crowding — the tooth pushes neighbors out of alignment, undoing orthodontic work
- Decay or gum disease — partial eruption traps bacteria the toothbrush can't reach
- Cysts or damage to nearby teeth
- Recurrent pain or infection in the back of the mouth
If your dentist or orthodontist has flagged any of these, an oral surgery consult is the next step. We'll review your panoramic X-ray, take a 3D scan if needed, and walk you through whether extraction is the right call.
What "same-day" actually means
It means we can complete your consult and your extraction in the same visit when the case is appropriate. Some cases qualify, some don't. Generally we can do same-day when:
- Recent imaging is available, or we can take it on site
- The roots aren't wrapped around a critical nerve
- You've fasted appropriately if sedation will be used
- You have a ride home after
If any of those aren't in place, we'll schedule the surgery date — usually within days, not weeks.
Anesthesia options
Wisdom teeth can be removed under local anesthesia alone, local plus nitrous oxide, or IV sedation. Dr. Perez-Rosich will recommend the right level for your case, your age, and your comfort. Most patients with impacted teeth choose IV sedation; most patients with simple eruptions don't need it.
Recovery — the real timeline
- Day 1: soft foods, ice, prescribed pain management. Most patients sleep most of the afternoon.
- Days 2–3: peak swelling. This is normal.
- Days 4–7: swelling subsides. Most patients return to work or school.
- Week 2: routine post-op check. We'll confirm the sites are healing well.
Recovery is faster than most people expect when the surgery is done by an experienced oral surgeon. The difference between a textbook recovery and a complication is often surgical technique.
